尊敬的某某老师您好：

在这次数学考试中，我遗憾地未能完成及格要求，给班级数学考试拉低了平均分，本人感觉到相当难过，也相当难堪。数学一直是我在努力学习的功课之一，本着努力就会有回报的心态学习它，但是总会在考试中错失。此次考试失败，证明本人用功依然不足，方法依然不对。在向老师道歉的同时，本人也承诺，要好好学习数学，改正之前错误的学习态度，纠正个人的在数学方面的错误方法，也希望向数学成绩好的同学多学习，向他们请教，也请老师不要放弃我，给我更多的关心和支持，相信在老师和同学们以及我个人的努力下，我定会在下次考试中，取得好成绩。

请老师相信我，也请老师接受我这次深深的歉意。事已到此，说再多的话也没有意义，请老师看我的努力，看我以后的成绩。再次感谢老师辛苦的教学，并希望老师多多督促本人学习，严格要求我，以共同提升数学成绩为目的。谢谢老师。
